Please fill out one form for each baby, child & teenager in your family who will attend UU Nashua services and programs this year.

We need families to sign up each year so we can stay up to date and better support and plan for our faith formation ministry.

For events when parents/guardians drop off children or youth for a program, we will have additional permission slips and health forms for you to sign. On Sunday mornings, we ask parents/guardians to stay on the church campus or notify staff if you will be unable to stay.

Photo Release: By registering, you are allowing your child(ren) to participate in activities that may be photographed or videotaped in a building that regularly hosts public events. Names will not be used along with any child's photograph. Do we have your permission to include your children's photograph in any of our church publications?

(4 times a year) Attend middle school or high school special event outings. Attend Our Whole Lives class for k/1 on Sunday mornings Attend playground meet up on occassional Saturdays Join all ages UU nature walks. Join Coming of Age program for the year for 8th & 9th graders. 2nd & 3rd Sundays from 4-6 pm

On Sunday Mornings, how might your child / youth like to participate?

Help open the wonder box during Sunday worship services. Help act out a story for all Ages in worship Read a poem, reading or skit in church Play an instrument in church or children's chapel Sing in family choir or youth choirs Sing a solo or duet for a Sunday service. They would like to help on a smooth Sunday team with adults who help greet, host fellowship hour, rover and prep for Sunday services They would like to attend services in the worship space with the whole congregation They are not sure yet

On Sunday mornings, which faith formation program would you like to sign up for - all are drop-in and we encouage all to come when they can. Attend the Children's Worship room - for 0-3 year olds. (offered every Sunday from 9:50-11:15ish) Join the authenitc connection teens group for games and conversation 2nd & 3rd Sunday Attend mixed ages children's chapel services for Pre K - 6th grader on the 1st Sunday of the month They would like to be helper in a younger classroom on Sunday mornings. Attend UU Values Workshop choices for Pre-K-5th graders on 2nd & 3rd Sundays of the month (choices are listeed in the following 3 choices... They would like Wonder Art projects - They would like community building games - they like to move and play with others. They would like to do Love in Action LEGO building - stories and lego building guided by UU values. They are not sure yet.

One more note...Shared leadership between staff and volunteers makes our faith formation program possible.

There are many ways to connect and engage with our faith formation program, even for the busiest families!

Faith Formation Director, Sadie Kahn-Greene, conducts background checks and trains all volunteers.

We always have two adults present with children and youth and have a thorough, safe congregation policy, which can be found on our church website.

While we are all pulled in many directions, connections with others (especially at church!) can be a gift and a blessing! We hope to get to know you as you join us in worship and events this year.

After you have been with us for a year, you will be eligible to volunteer in our faith formation children's program, and we look forward to supporting you in finding a meaningful way to connect!
