Please fill out one form for each baby, child & teenager in your family who will attend UU Nashua services and programs this year.
We need families to sign up each year so we can stay up to date and better support and plan for our faith formation ministry.
For events when parents/guardians drop off children or youth for a program, we will have additional permission slips and health forms for you to sign. On Sunday mornings, we ask parents/guardians to stay on the church campus or notify staff if you will be unable to stay.

Here are a few more notes about the shared leadership between staff and volunteers that make our faith formation program possible.
· There are many ways to connect and engage with our faith formation program, even for the busiest families!
· Faith Formation Director Sadie Kahn-Greene conducts background checks and trains all volunteers.
· We now have a team of three childcare providers on staff to support our program and special events: Rosie LeFrancois, Maneula Calderon, and Kristen Garuti. We will see two of them on Sunday mornings, offering care in the Rainbow Room before, during, and after church until 12:30, as needed.
· We always have two adults present with children and youth, and we maintain a thorough, safe congregation policy available on our church website.
While we are all pulled in many directions, connections with others (especially at church!) can be a gift and a blessing. We hope to get to know you better as you join us in worship and faith formation this year.
After you have been with us for a year, you will be eligible to volunteer in our faith formation children's program, and we look forward to supporting you in finding a meaningful way to connect!
